Vintage: The Merriespruit Tailings Dam Disaster
On the night of 22 February 1994, one of South Africa’s worst mining related disasters occurred when the No. 4A tailings dam at Harmony Gold Mine near Virginia in the Free State failed. The collapse released approximately 600 000 cubic metres of water and mine tailings, which swept through the nearby suburb of Merriespruit.
Earlier that evening, severe thunderstorms struck the area, with an estimated 50 millimetres of rain falling within about 30 minutes. The heavy rainfall caused the tailings dam to overtop. A section of the embankment then failed, releasing a fast moving flow of tailings and water. The wave travelled several kilometres from the dam and reached parts of Merriespruit, where some homes were located only a few hundred metres downslope.
17 people lost their lives and around 80 houses were destroyed. Many other properties were damaged, leaving widespread devastation in the community. The disaster remains one of the deadliest tailings dam failures in South African history.
Investigations found that the dam had experienced stability and seepage problems before the disaster. In March 1993, concerns about seepage on the northern wall led to the suspension of normal deposition activities in the affected compartment. Despite this, excess plant water containing small amounts of tailings continued to be discharged into the facility. Over time, water accumulated in areas where it should not have been, reducing the dam’s safety margin.
Evidence presented during the official inquiry showed that the dam did not meet the required freeboard standards needed to safely accommodate a severe storm. Satellite imagery taken shortly before the disaster showed water ponded against the northern wall of the dam. Witnesses also reported seeing water flowing over the crest of the wall on the evening of the failure.
A judicial inquiry concluded that the disaster resulted from a combination of heavy rainfall, overtopping, inadequate water management and operational failures. The mine and the contractor responsible for managing the facility were found negligent. The presiding judge described the dam as a “time bomb waiting to explode.”
